Why local search behaves differently here
San Diego is geographically spread and neighbourhood-oriented in a way that materially affects local search. Someone searching from La Jolla and someone searching from El Cajon get meaningfully different results for the same query, because proximity weighs heavily. A single county-wide ranking position does not really exist.
Practically, that means a business with one location cannot expect to dominate the whole county, and should not be sold that outcome. What is achievable is strong visibility in the areas you genuinely serve, which for most businesses is where the revenue is anyway.
The sectors where this matters most
Healthcare and dental. San Diego has a dense concentration of private practices competing for the same patients, and patients research heavily before booking. Professional services. Attorneys, financial advisors and consultants get searched by name before every engagement. For regulated professions, a disciplinary record or an old complaint site can rank for years and is disproportionately damaging.
Home services. Contractors, HVAC, plumbing and landscaping compete almost entirely on local pack visibility and star rating. A drop from 4.6 to 4.1 is a measurable revenue event.
Hospitality and tourism. Businesses serving visitors live and die on review platforms, and are unusually exposed to seasonal review clustering — a bad summer weekend can skew a rating for months.

A note on location pages
You will see agencies produce fifty near-identical pages, one per suburb, with the place name substituted. This stopped working years ago and now risks being treated as exactly what it is. Two or three genuinely distinct pages about areas you actually serve will outperform fifty thin ones — and they will not put your domain at risk.
